The first is a very brief tactile examination of the first upper premolars (cheek teeth). This examination only provides information about the condition of these teeth. If an Oral Adjustment (OA) is not indicated here, I would proceed to a full examination of every tooth in the mouth. This requires sedation, a full mouth speculum, light source, equine dental mirror, periodontal probes, and dental explorer, to complete a visual and tactile examination of each tooth in the horse’s mouth. If the doctor does not have these tools or does not use them routinely, they are not doing a quality examination and the result is a substandard job or even injury to the horse! The first examination only tells us if an oral adjustment is needed, it does not tell us one is not needed, because only a small portion of the dentition can be evaluated this way!</image:caption>

Does it live alone or with other horses and if so how many? Do the other horses on the farm travel to events that expose them to horses from the surrounding area or from far away? Has the horse been ill or stressed in a way that could compromise its immune system? Has it had any medications that may render vaccinations ineffective or dangerous for the horse at the current time? Has the horse ever been vaccinated for the disease before and if so how long ago? Has the patient had an adverse reaction to a particular vaccine in the past? These are only a few of the more obvious considerations I make when deciding what vaccines a patient requires. There is no perfect vaccination plan to put all horses on. A trained professional should evaluate each patient’s needs on an ongoing basis and make an informed decision annually as to what to vaccinate a patient for.</image:caption>

      <image:caption>Having a new foal is an amazing and exciting experience, but also comes with a lot of responsibility. Evaluating foals between 12-24 hours after birth is extremely important for their health. Foals need to drink quality mare’s colostrum before 18-24 hours of age or risk developing failure of passive transfer. Foals that develop failure of passive transfer require plasma transfers and are at increased risk of developing septic joints, systemic sepsis, gastrointestinal dysbiosis, and other deadly diseases. By evaluating the mare’s colostrum and the foal’s ability to nurse appropriately, steps can be taken to mitigate these concerns. Foals are also evaluated for evidence of broken bones, a ruptured bladder from the birthing process, patent urachus, gastrointestinal obstruction (such as retained meconium), gastrointestinal diseases, any developmental abnormalities and more during this examination. It is also important to understand that foals oftentimes have conformational abnormalities that are significantly easier to correct when identified and treated early on. Something as simple as bandaging in the first few days or weeks of life may correct a conformational abnormality that will require a surgical correction at an older age.</image:caption>

      <image:caption>Digital Radiography is a key diagnostic tool in identification and treatment planning of dental disease, especially when it comes to the early stages of a disease process. Imaging helps us to evaluate the affected and surrounding structures and tailor the best possible treatment plan for your horse.  KMEH has state-of-the-art, specialized digital radiographic equipment allowing us to take both intraoral and extraoral radiographs. The addition of intraoral radiographs provides a means to obtain oral radiographic images of specific oral structures that are difficult to evaluate from extraoral images. Intraoral digital radiographs help eliminate confusing overlapping structures from areas other than the target tissues (teeth, etc.), providing “cleaner” images and allowing for more detailed evaluation of the structure(s) of interest.  Oroscopy is endoscopy of the mouth, or video feed of a camera image from inside the mouth. It is used to obtain detailed images of less accessible portions of a horse’s mouth, such as the back of the mouth, or where the tongue or cheek runs along the teeth. It can also be used to provide detailed, magnified images of the occlusal surface (where the top and bottom teeth pass over each other to grind food) to further investigate suspected abnormalities, such as infundibular cavities. Oroscopy may be recommended as an additional way to examine your horse’s mouth, as a way to increase visibility for completion of delicate intraoral procedures, or as a way to track and record progress of a staged treatment plan or the healing process. Sinoscopy is an endoscopic exam (camera view) of the sinuses. Horses’ maxillary (upper) teeth are intimately associated with the sinus cavities of the skull (specific teeth are associated with specific sinus cavities). Infections of the maxillary tooth roots can lead to sinus infections (secondary sinusitis), and sinus infections can, in turn, affect the maxillary teeth. Sinoscopy is used to obtain detailed images and information about disease processes that affect the sinus cavities and evaluate for dental involvement. This information helps us determine specific diagnoses and plan surgical procedures and treatment plans.</image:caption>

      <image:caption>EOTRH (Equine Odontoclastic Tooth Resorption with Hypercementosis) is a progressive painful disease of primarily the incisor and canine teeth of the horse. It may occur in a number of presentations including: primarily resorptive where the teeth are eaten away over time, hypercementotic where the teeth form excessive amounts of cementin over their surfaces, or a combination of the two. Often the horse will hide its pain very well and as an owner you may not realize your horse is suffering. Tests like the “carrot test” and evaluation of the “grimace response” may be helpful to assess levels of pain. Unfortunately, to date the best treatment for EOTRH is extraction of the affected teeth.  This may be an involved surgery requiring an experienced dental surgeon with intimate knowledge of the involved anatomy. Specialized surgical instruments and techniques are employed to achieve the best possible outcomes. These may include raising tissue flaps and removal of bone to reshape the upper and/or lower facial bones. The good news here is that horses generally do extremely well postoperatively and go on to live a happy pain free life.       <image:caption>Horses have six pairs of paranasal sinuses. When a sinus becomes infected or affected directly it is termed “primary sinusitis or primary sinus disease”. When a sinus becomes infected or affected due to extension of disease from an area outside the sinus it is termed “secondary sinusitis or secondary sinus disease”. There are a number of treatment options for sinus infections depending on the source of the problem. In some cases tubes may be placed under the skin and through the bone into a sinus to allow flushing of the sinus for a few days. In other cases the sinus may need to be opened surgically to allow removal of dead and decaying tissues, cysts, or infected teeth or tooth roots. Masses may form in the sinuses requiring one or more sinuses to be surgically opened to gain access to the mass for safe biopsy or for removal etc.. Horse sinus anatomy is complex and involves a number of critical vessels, nerves and other structures like the thin wall of bone that separates the sinuses from the area housing the brain. Special training and advanced care techniques must be employed when performing sinus surgeries.       <image:caption>Colic is abdominal pain and has a wide variety of causes. Common signs of equine colic are inappetence, pawing, camping out, flank watching, biting at the flank or sides, “sucking up” of the abdomen in the flank region, decreased fecal output, rolling, sweating, etc. Colic has a wide variety of causes and can be mild and self resolve, severe and require surgery, or anywhere in between. Our veterinarians can provide ambulatory colic care in the field and medical colic care with fluid therapy in the hospital.       <image:title>Emergency Services - Lacerations</image:title>
      <image:caption>Lacerations are a common injury in the equine world. It is important to remember that a small laceration can still be an emergency depending on their location and what the injury occurred on. Lacerations around joints should be evaluated for joint involvement so that necessary treatment to prevent or treat joint infections can be initiated as soon as possible. The sooner a laceration can be repaired, the better its rate of healing and the less it will scar. Not all lacerations can be sutured closed and other methods of repair, such as bandaging, will be evaluated on an individual basis. Tetanus Toxoid vaccine boosters are often necessary in laceration cases. Knowing your horse’s vaccination history and when it last received a tetanus toxoid vaccine is useful information.</image:caption>

      <image:title>Emergency Services - Eye Emergencies</image:title>
      <image:caption>Any injury to the eye or eyelids is an emergency due to the risk of loss of vision. Injuries to the eye itself that are left untreated can develop into infections of the eye that are difficult and intensive to treat. Whether your horse's eye is squinting, tearing, swollen, has something weird floating in it, or is abnormally red around the eye or the tissue that surrounds it, it should be seen by a veterinarian. Eyelid lacerations should also be seen on an emergency basis to try to save the protection it provides to the eye. Eyelid lacerations that are left untreated or receive delayed treatment often experience dieback or loss of some or all of the affected eyelid tissue.</image:caption>

      <image:title>Emergency Services - Esophageal Obstruction Relief</image:title>
      <image:caption>Esophageal obstruction, commonly referred to as “choke”, is when a food bolus gets stuck in a horse's esophagus. While this does not actually prevent a horse from breathing, it can be very painful and can risk permanent damage to the esophagus and aspiration pneumonia if left untreated. Horses that are going to self-resolve a choke usually do so within the first 30 minutes of experiencing the esophageal obstruction. If they have not resolved the “choke” by then, they are unlikely to and need emergency medical attention. Common signs of esophageal obstruction are food material coming out the nostrils, inability to swallow, extended neck, tearing, coughing, distressed behavior. If you observe any of these signs in your horse, please call us right away.</image:caption>
